Male Infertility and Andrology

Azoospermia (i.e. - no sperm) is the medical condition which affects about one in a hundred men where there are no measurable levels of sperm in the semen. This condition is associated with very low levels of fertility.
The conditions result in no sperm being in the ejaculate:
• No sperm is being produced by the testis.
• Occlusion, whilst sperm is produced, there is a blockage stopping the sperm going to out.
• Retrograde ejaculation- where sperm passes into the bladder at ejaculation rather than out.

Other causes include hormonal imbalances, chromosome abnormalities and a special type of carrier status for Cystic Fibrosis.

Most men due to undergo semen analysis fear the diagnosis of azoospermia. However, such diagnosis does not mean that the man produces no sperm, or can never be made to produce any sperm therefore will never have his own biological child. Even then if there are no sperm in the ejaculate, sperm can often be harvested and used to achieve fertilization.
Depending on the cause, treatment is either by treating the cause such as removing an obstruction or hormone treatment, or by retrieving sperm for use in IVF techniques. Obtaining sperm from the testis is done by procedure under local or general anaesthetic. The sperm is then used during fertility treatment, called ICSI (intracytoplasmic sperm injection). During the ICSI procedure a single sperm is injected into the egg to fertilise it and create an embryo.

Vasectomy Reversal

Mr Almallah also specialises in vasectomy reversal which in the majority of cases is the most effective first option for a couple wishing to conceive after the man has undergone a vasectomy. It is the least invasive and most cost effective method of conception. If the procedure is successful, the man’s sperm may be of adequate quantity to allow for natural conception.
